Content of the declarations

C.1 Annex II — Due diligence statement

1. The operator's name and address and, in the case of relevant commodities and products entering or leaving the market, the EORI number.
2. The Harmonised System code, a free-text description including the trade name and, where applicable, the full scientific name, and the quantity of the product the operator intends to place on the market or export. For products entering or leaving the market, the quantity is expressed in kilograms of net mass and, where applicable, in the supplementary unit set out in Annex I to Regulation (EEC) No 2658/87; in other cases, in net mass with an estimate or percentage of deviation or, where applicable, in volume or number of units.
3. The country of production and the geolocation of all plots of land where the relevant commodities were produced; for cattle and for products fed with relevant products, all the establishments where the cattle were kept.
4. (Deleted.)
5. The wording: "By submitting this due diligence statement the operator confirms that due diligence in accordance with Regulation (EU) 2023/1115 was carried out and that no or only a negligible risk was found that the relevant products do not comply with Article 3, point (a) or (b), of that Regulation."
6. The signature in the format: "Signed for and on behalf of: / Date: / Name and function: / Signature:".

C.2 Annex III — Simplified statement (MSPO)

1. The name and address of the micro or small primary operator and, where it imports or exports, the EORI number.
2. The HS code and a free-text description including the trade name, and the single estimated annual quantity it intends to place on the market or export, under the same unit rules as Annex II.
3. The country of production and the geolocation of all the plots, or the postal address of the establishment or of the plots; for cattle, all the establishments where they were kept.
4. The wording: "By this declaration the micro or small primary operator confirms that it will exercise due diligence in accordance with Regulation (EU) 2023/1115 in relation to the relevant products that it places on the market or exports, and that it will place them on the market or export them only where no or only a negligible risk was found that the relevant products do not comply with Article 3, point (a) or (b), of that Regulation."
